Deaths in GR

I know what you mean about Bianca.  I left her out because there seemed 
to be a bit too much ambiguity to make the call.  I took "for sure" to 
mean that the "narrator" tells us someone's dead.  Slothrop, stressed 
out, paranoid, bumping around in the dark, "thinks" he sees Bianca 
hanging there out of the corner of his eye.  Later (p. 576) he remembers 
her "dead flesh," but this is never verified by the narrator.
