I used to have a copy online but there is a problem with copyright,
which I believe belongs to Jules for the electronic version and
Playboy for the printed version. I don't want to get (myself or my
institution) involved in any legal battles over copyright so I deleted
the online version. Jules intends to place a copy on his web server,
to which I will add a link from my miscellania page when it arrives.
