Interesting, you're right -- my 1990 Random House (Vintage imprint) has
the index, while the 1969 McGraw-Hill does not.  Index is credited to
Vivian Darkbloom (aka VN) so I assume he added it himself...?  (The 1990
edition btw has a much nicer cover!)
