Willis says "Trust me, I'm an osteopath", and Jimmy mistakes that as an
admission of some sort of religious affiliation (i.e., Jimmy thinks
Willis is saying he's trustworthy because he's a religious man). So
Jimmy replies, "It's OK, we're open-minded, couple of boys in the
outfit are evangelicals ... " Boom boom. It's a pretty lame joke. (But
the "Trust me, I'm an osteopath" line has a nice sense of the absurd
about it.)
The "pinkinroller" insult comes before that card is on the table I
think. The sense I had was that Jimmy only notices Willis again when he
says "I can fix that". Jimmy looks up, sees Willis approaching, sees
what he thinks is a roller from a pinking tool in Willis's hand, and
says what he says. I think that, if Willis was still waving the gun
around as he approached, Jimmy's response would have been quite
different. And I doubt that Willis would want to hold the gun for any
longer than necessary. But it's certainly plausible that Jimmy's insult
refers back to the gun ("great big pistol" -- sarcastic), and/or to
Willis ("little lady" -- sarcastic) as the holder of that gun.
And, admittedly, Willis doesn't seem to have anything at all in his
"lilywhites" when he fixes Jimmy's back. On that basis, I think Paul's
reading probably is the best fit to the text.
