Do you know Mick Farren? I'd suggest asking him as to his  knowledge 
on this question. I suspect the phrase evolved out of earlier  
formulations - rock, dope and fucking in the streets and the like.  
Mick in his tenure at International Times was probably familiar with  
variations on the theme. Underground cartoonists or early Rolling  
Stone journos might be another source - Skip Williamson cartoons come to  
mind.
