ATD: expanded ToC

[Monte Davis]

I've prepared an expanded table of contents. It lists the opening words and
page# for each of the 70 "chapters" -- blocks beginning with a large gray
capital letter, to which I've assigned serial numbers. Within those, it
lists the opening words and page# for the "quick cuts" -- segments beginning
with a two-line break and small caps.

It looks like this in tabbed columns (you may need to turn off word wrap):

It runs 11 pages single-spaced, a 100K MS Word document. I'm finding it
helpful in understanding the book's pacing and its switchers among story
