Frank Norris, _McTeague_ (dentistry).  Does _Johnny Got His Gun_ count?

In Tobias Smollet's epistolary novel _Humphrey Clinker_, there are some 
wonderful letters written from curmudgeonly Matt Bramble to his physician, 
Dr. Lewis, which capture the essence of 18th C medical treatment (in all its 
hilarity).

Don't forget ol' Roger Chillingworth in _The Scarlet Letter_, or Joseph 
Heller's Doc Daneeka in _Catch-22_.

Finally, there's a very interesting use of medical imagery used in the 
dialogue of the angel Michael in Books 11 and 12 of Milton's _Paradise 
Lost_.  While it's not a characterization of a "doctor" per se, nor does it 
address the medical profession directly, it might be worthwhile to see how 
Milton uses the image of medicine at this particular part of the epic.
