It's "difficult quite to distinguish which" of the three categories 
("Fops, Macaronis, or Lunarians") the group is (21). In other words, 
the three terms are being used more or less interchangeably. They're 
English dandies, but I think that part of the point in juxtaposing the 
three terms in such a way is that the culture and social climate have 
generated so many of these types of gents (who dress up in fancy 
clothes and preen and put on airs and mock others) that sub-genres have 
been invented.

A hanger is indeed a short sword which hangs from the belt.
