We (in Italy) usually use it to indicate lateral doors of cars (though
it maybe a regional use of the word) and in other very different
contexts. For example there are "sportelli" in the banks or in post
offices, where you can pay bills, send mail and so on. I think the
english for this usage is just front office.

Anyway I've never known anybody with such a surname though it seems
there are some Sportellos in Italy, mainly located in Apulia (Puglia).
