I think your first hypothesis is better than the second one. But I'd  
also say that the postcard can represent another place than the  
island from where it was actually sent. There could be a link between  
both places, and Shasta may also have not been to this underwater  
city. It could be a fantasy postcard that was given to her on this  
Pacific island. It makes her message more ambiguous, and it helps Doc  
finding a secret message behind it: she's in a bad condition and he  
has to investigate on the site she's reminding him.
One more thing that makes "besides" no typo: p. 163: "a voice from  
somewhere tells you you have to be," and the picture is a photo of  
some other place than where she is. It makes the whole quite consistent.
