I was lucky enough to find a signed edition ($10!), hardcover, published by Sinclair-Stevenson Ltd.
Tore Rye Andersen sent me an excellent article about Norfolk that he wrote for Gale's British Writers. According to Andersen, the first American edition was published by Harmony Books, which, at the editor's insistence, was heavily cut. Norfolk at first defended, but later opposed this cut version. Grove Press issued an uncut version in 2003, as a result.

That being said, the tidy ending was probably my least favorite part of the book. But if Norfolk wrote it that way, it belongs there.

did you read the US or British edition? A friend told me that the British edition is 150 pages longer and provides a lot of plot answers that the US edition ignores.
