It really does on one hand seem odd for Derek to have
been offended by being called by his last name (though
it certainly can seem a bit callow, a bit rude, to be
called solely by your last name, no matter what your
last name is), and it really does seem a bit of an
interpretive leap from one's actual last name to a
putative slur (then again, cf., "That's os-Wee-pay"). 
But "intent" is beside the point here.  There was
perhaps a certain insensitivity involved, esp. in a
milieu where many are likely to eb aware of the
particular connotation(s) of "Maus" with which Derek
took offense.  But the real issue is, just how badly
has this milieu degenerated so that such an
interpretative leap is not only a distinct
possibility, but is actually made?  "Intent,"
shmintent, certain responsibilities were not taken
nonetheless, either before (again, sensitivity) or
after (here, apologies) the event.  This sort of
situation is of course endemic here ...
