"This was last set ..." in all editions that I have access to.

>2.  This is either a typo, an obscure jazz term or an oddly gratuitous pun:
> p. 306: "They took fours with the drummer, stated the main thyme in unison and left the stand."  Thyme could be a cutesy rendering of "time," but it's just a typo rendering of "theme."
