On M&D cover and ampersand:

Warren St. John, detailing the way in which TRP was involved in the

As for the dedicaton: young Jackson was at the time learning to read, I 
think - the boy was 7 at the time. So you can read the dedication as: 
for Melanie (now) and Jackson (later). My 2 euro cents.
